CRDS Project Request Form

  • This commitment letter must be filled out in its entirety and submitted with payment as indicated below. Design work will not begin until this form has been completed. Once completed, please submit information on the physical property of your facility. Please include: property lines, setbacks, street and entrance locations, dimensions of facility, location of doors, location of interior walls, permanent structures (restrooms, offices, floor drains, etc.) A grid is available to complete your facility diagram if needed. Please contact the CRDS Manager to have a grid sent out to your facility. Please allow 6-8 weeks before receiving drawings.
  • In consideration for the services to be performed by Collision Repair Design Services, ("Customer") hereby agrees:

    • To Pay the applicable fee for the presentation of design and layout recommendations made to customer. The fee for the presentation is: Eight Hundred Ninety Five ($895.00) U.S. Dollars. This fee shall be paid in full upon execution of this Letter and is money back guaranteed if customer is not satisfied with layout and design services. Make checks payable to :
